This workspace generates narratives and medical necessity statements in your browser. It is publicly accessible and does not require an account.

Encounter information

The app does not send form entries or generated narratives to a server, save them in browser storage, or include analytics or external fonts. Entries remain in the open browser tab until you clear the form, reload, or leave the page. Provider details remain during form resets and are cleared on reload.

Copying and clearing

Copy buttons place text on your device’s clipboard. Clearing the form removes encounter entries, treatment selections, and generated statements in this tab. It does not clear your device’s clipboard or anything you pasted elsewhere.

Website delivery

Netlify delivers the website and may process ordinary connection information, such as your IP address, to serve it. Encounter information is not part of the website request. Anyone using this tool should follow their agency’s documentation and device policies.
